    DOCUMENTS REQUIRED FOR REGISTRATION 

    The following items are needed at the time of registration:

     Proof of Age

    • Certified copy of a birth record
    • Statement by the local registrar or a county recorder certifying the date of birth
    • Baptism certificate duly attested
    • Passport
    • Affidavit of the parent, guardian, or custodian of the minor
    • Other appropriate means of proving the age of the child

    Up-to-date immunization record
    Physical exam
    Dental checkup
    TB skin test with the results
    Valid parent identification card (example: drivers license or other ID card)
    Proof of current address - Reasonable evidence of residency for a pupil living with his or her parent or legal guardian shall be established by documentation showing the name and address of the parent or legal guardian within the school district, including, but not limited to, any of the following documentation:

    • Property tax payment receipts;
    • Rental property contract, lease, or payment receipts;
    • Utility service contract, statement, or payment receipts;
    • Pay stubs;
    • Voter registration;
    • Correspondence from a government agency;
    • Declaration of residency executed by the parent or legal guardian of a pupil. 

    Spaces are limited in some district schools.
